Past simple form doesn't require any additional words. All that has to be done is to add -ed or -d to the base form of the verb. However, some verbs are irregular, in that simply adding -ed or -d is improper. Past continuous form involves an activity that occurred before the present time and continues. Here the auxiliary verb (was/were) come into play. Past perfect form involves a situation spoken of that had finished occurring the past and includes the auxiliary verb (has) or (has not). Past perfect continuous form involves an activity that had been occurring for some time, but stopped before the moment being spoken of. This includes the auxiliary verb (had been) or (had not been).
